Healthcare services are funded through Medi-Cal, Medicare, IHSS, the utilization of a home health agency based on the individual’s healthcare plan and most importantly the individual’s primary physician. If an individual requires a health care plan, United Supportive Living’s RN shall assist the primary physician in developing the plan. The healthcare plan shall then be part of the individual’s IPP so that it can be monitored, tracked and most importantly followed.
United Supportive Living shall strive to be at the forefront of progressive healthcare for the individuals we serve. Our quest is to utilize the extensive list of specialists, so that our individuals may with assistance, support and guidance achieve optimum healthcare while receiving services from our agency. This shall also include instructional support to the individual.
By utilizing an RN as part of our agency’s personnel and partnering with a home health agency based on the individual’s health plan, United Supportive Living shall be able to accept and provide the medical care and supervision of individuals with the following health conditions:
- Diabetes
- Hypertension
- Gastrostomy care
- Staph infection or other serious infection
- Administration of oxygen
- Ambulatory issues
- Cancer
- Pregnancy
- Heart conditions
- Enemas, suppositories, and/or fecal impaction removal
- Incontinence of bowel and/or bladder
- Injections
- Intermittent Positive Pressure Breathing Machine
- Stage 1 and 2 pressure sores (dermal ulcers.)
- Wound care
- Or any other specialized medical condition that would fit in our range of care
United Supportive Living shall partner with a home health agency based on the individual’s health plan and thus provide the required after hospital stay recommendations that require any type of medical or physical care that the RN cannot complete due to specialty, such as physical therapy. The RN shall be initially employed on a part-time basis and shall monitor and train the SLS instructor and individual in their specific health care condition.
