United Supportive Living shall along with the individual’s circle of support assist the individual and/or their authorized representative with making informed choices, directing their life, their services and their future. United Supportive Living shall serve as the coordinator for all medical, dental and psychiatric appointments. We shall serve as the liaison for the transmission of feedback to the various medical and psychiatric personnel for each individual.
United Supportive Living shall be an advocate for the individual and/or their authorized representative to ensure their rights under Title 17 are not violated or diminished in any manner. United Supportive Living shall also ensure the individual’s objectives in their ISP are followed. This process shall take place during the quarterly or semi-annual meetings to discuss the progress of the objectives and any issues that may have risen since the last meeting.
