United Supportive Living along with each individual and/or their authorized representative and the individual’s circle of support shall create an ISP that is flexible and tailored specifically for their needs and services. Each individual’s ISP shall be reviewed quarterly; however, it can be changed and/or modified at any time based on the changing needs, desires, or condition. Individuals shall not be denied eligibility for supported living services solely because of the nature and severity of their disabilities.
United Supportive Living shall tailor the supported living services that each individual requires based on their behavioral, psychiatric and medical supports combined with their circle of support recommendations. United Supportive Living shall always attempt to combine services when two (2) or more individuals reside together, such as overnight staffing hours. Based on these recommendations SLS instructors shall be assigned on the following ratios to the supported individual.
